You need a new master cylinder (go with the Mopar Performance aluminum part unless you want the OEM look), rotors and calipers? Some calipers will mount to your drum spinble (SSBC), some will require a new spindle.
The best kit for a stock-like setup is to use the 73-76 A-body spindle and post-73 brake parts (which you can get from parts stores). You can upgrade on this kit by using 11.75" discs (1976 and later B-body)
The spindle is reproduced, and can be found at Master Power Brakes, for example. They can source al parts for a complete kit. It's never a bolt-on though, you'll have to make your own lines to the master cylinder.
The calipers are heavy single-piston pieces.
If you want to go aftermarket, SSBC has great kits that will fit under 15" wheels. I've seen one on a friend's 68 Charger, it's a very nice setup, and (much) lighter than the OEM parts.
AR Engineering also makes some bigger kits, like viper calipers on 11.75 or 13" rotors.
Doctor Dif (a resident expert on moparts) also has similar kits, with Brembo discs and calipers.
And then there's XV Motorsports (prepare your money though).
All this from around 700 bucks all the way to 3500.
